約翰·Q是個工人，工作賣命也很顧家，可生活依然不富裕。為養家糊口，妻子丹妮斯也要上班掙錢，夫妻倆因忙碌感情日漸淡漠，婚姻幾近破裂。他們生活中唯一的亮點，就是9歲的兒子邁克爾。
可不幸總是出其不意地降臨到苦命人的身上。一天，邁克爾突然昏到在棒球賽場上，約翰·Q把他送到醫院後得知：兒子患有嚴重的先天性心臟病，需要進行心臟移植手術才能保住性命。

心臟移植手術的費用非常昂貴，而約翰·Q的醫療保險遠不足以負擔15萬美金的手術費。醫院的頭頭得知這個情況後，決定把邁克爾從等待移植者的名單中剔除，除非約翰·Q能預付30%的手術費。此舉激怒了約翰·Q，在求助於邁克爾的主治醫生特納未果的情況下，他憤然持槍闖入了醫院的急診室，綁架了幾位醫護人員和病人做人質，要求院方立刻為兒子動手術。

然而事情並不像約翰·Q想像的那麼簡單，面對他的不僅是一群無辜又需要護理的人質，還有一個經驗豐富的談判家和一個想要速戰速決的警長。約翰·Q和警方的對峙還驚動了媒體，全市的大眾對這一事態的發展都投以了極大的關注。他們的感情似乎更多傾向於違法者約翰·Q，因為他是當今不近人情的醫療和保險制度的受害者。但大眾的同情並不能幫助約翰·Q，他的前途到底如何呢？

丹澤爾·華盛頓憑《光榮》一片獲奧斯卡最佳男演員獎的黑人影帝近年來在銀幕上一直佳作不斷：《人骨拼圖》、《沖鋒陷陣》、《颶風》、《訓練日》都是由他領銜主演的，不僅多產而且還能保證高質，2000年的《颶風》為華盛頓贏得了金球獎和奧斯卡提名，今年《訓練日》又使他再度入圍奧斯卡最佳男主角提名，並成為拉塞爾·克羅的最有力的競爭者。

除了主角之外，杜瓦爾是本片中另一位奧斯卡影帝，他的獲獎作品是1983年的《溫柔的憐憫》一片。此外，杜瓦爾的名作還包括《教父》、《現代啟示錄》、《傳教士》和《民事訴訟》等等，以上影片均為他贏得過奧斯卡的提名。

扮演醫院頭頭的安妮·海切是這一兩年裡竄紅的女星，《六天七夜》、新版《精神病人》、《搖尾狗》、《忠奸人》等影片里都有她的身影。不過，也許人們更津津樂道的不是她的演技，而是她和她的同性及異性戀人的分分合合。

導演尼克·卡薩維茨是已故著名演員兼導演約翰·卡薩維茨之子，畢業於美國著名的戲劇藝術學院（American Academy of Dramatic Arts），尼克繼承了父親的天賦，是一位既能導又能演的全才。1997年的戛納電影節上，尼克根據父親遺作劇本執導的《戀戀風暴》曾為西恩·潘摘得了最佳男演員大獎；作為演員，卡薩維茨參加過艾迪·墨菲的《生活》、約翰尼·德普的《宇航員的妻子》、吳宇森的《變臉》等影片的演出，可謂導、演雙豐收。
