约翰·Q是个工人，工作卖命也很顾家，可生活依然不富裕。为养家糊口，妻子丹妮斯也要上班挣钱，夫妻俩因忙碌感情日渐淡漠，婚姻几近破裂。他们生活中唯一的亮点，就是9岁的儿子迈克尔。
可不幸总是出其不意地降临到苦命人的身上。一天，迈克尔突然昏到在棒球赛场上，约翰·Q把他送到医院后得知：儿子患有严重的先天性心脏病，需要进行心脏移植手术才能保住性命。

心脏移植手术的费用非常昂贵，而约翰·Q的医疗保险远不足以负担15万美金的手术费。医院的头头得知这个情况后，决定把迈克尔从等待移植者的名单中剔除，除非约翰·Q能预付30%的手术费。此举激怒了约翰·Q，在求助于迈克尔的主治医生特纳未果的情况下，他愤然持枪闯入了医院的急诊室，绑架了几位医护人员和病人做人质，要求院方立刻为儿子动手术。

然而事情并不像约翰·Q想象的那么简单，面对他的不仅是一群无辜又需要护理的人质，还有一个经验丰富的谈判家和一个想要速战速决的警长。约翰·Q和警方的对峙还惊动了媒体，全市的大众对这一事态的发展都投以了极大的关注。他们的感情似乎更多倾向于违法者约翰·Q，因为他是当今不近人情的医疗和保险制度的受害者。但大众的同情并不能帮助约翰·Q，他的前途到底如何呢？

丹泽尔·华盛顿凭《光荣》一片获奥斯卡最佳男演员奖的黑人影帝近年来在银幕上一直佳作不断：《人骨拼图》、《冲锋陷阵》、《飓风》、《训练日》都是由他领衔主演的，不仅多产而且还能保证高质，2000年的《飓风》为华盛顿赢得了金球奖和奥斯卡提名，今年《训练日》又使他再度入围奥斯卡最佳男主角提名，并成为拉塞尔·克罗的最有力的竞争者。

除了主角之外，杜瓦尔是本片中另一位奥斯卡影帝，他的获奖作品是1983年的《温柔的怜悯》一片。此外，杜瓦尔的名作还包括《教父》、《现代启示录》、《传教士》和《民事诉讼》等等，以上影片均为他赢得过奥斯卡的提名。

扮演医院头头的安妮·海切是这一两年里窜红的女星，《六天七夜》、新版《精神病人》、《摇尾狗》、《忠奸人》等影片里都有她的身影。不过，也许人们更津津乐道的不是她的演技，而是她和她的同性及异性恋人的分分合合。

导演尼克·卡萨维茨是已故著名演员兼导演约翰·卡萨维茨之子，毕业于美国著名的戏剧艺术学院（American Academy of Dramatic Arts），尼克继承了父亲的天赋，是一位既能导又能演的全才。1997年的戛纳电影节上，尼克根据父亲遗作剧本执导的《恋恋风暴》曾为西恩·潘摘得了最佳男演员大奖；作为演员，卡萨维茨参加过艾迪·墨菲的《生活》、约翰尼·德普的《宇航员的妻子》、吴宇森的《变脸》等影片的演出，可谓导、演双丰收。
